By requesting an ultrasound with Sweet Blessings Ultrasound LLC, you confirm that the ultrasound will be performed by a sonographer affiliated with Sweet Blessings Ultrasound LLC. You also confirm that you are currently under the care of a healthcare provider or midwife for prenatal care. The elective ultrasound being requested is a limited scan intended to confirm the heartbeat of the baby, the number of babies, position, placental location, and gender, when applicable. This ultrasound is not to be used in any way as a substitute for a complete diagnostic ultrasound under the care of a healthcare provider or midwife. Sweet Blessings Ultrasound LLC assumes no responsibility for failure to report any complications with the pregnancy beyond the above stated issues. Specifically, by way of limitation, structural deficiencies, developmental deficiencies, and/or other health abnormalities are not issues that will be addressed during this ultrasound. This is a non-diagnostic scan. Gender predictions are not 100% guaranteed, and Sweet Blessings Ultrasound LLC is not liable for an inaccurate gender prediction. These are elective ultrasounds only.
You understand that image quality can be affected by a variety of circumstances, including but not limited to the baby’s position, maternal body habitus, BMI, placental location, and the amniotic fluid index. Sweet Blessings Ultrasound LLC will make the best attempt to capture quality images of your baby; however, no guarantee can be made regarding image quality. Payment compensates Sweet Blessings Ultrasound LLC for the limited elective ultrasound completed by our sonographer.
